WAIVER & RELEASE OF LIABILITY FORM

I hereby declare that I understand and assume all risks involved in participating in the submission grappling tournament called WARRIOR CAGE GRAPPLING, including but not limited to, the risk of serious injury, paralysis, or death. I further acknowledge that I will be competing at the host location venue, secured by WARRIOR CAGE GRAPPLING, at my own risk. I understand that the sport of Jiu Jitsu Submission Grappling within WARRIOR CAGE GRAPPLING and related activities, may cause bruising, sprains, fractures, or other injuries to me. At the same time, I also declare myself to be physically capable of performing in the tournament and free from any disease that may prohibit me from strenuous activities. 

I also declare I have appropriate personal primary medical insurance fully covering any injury I may sustain. I fully assume these risks and waive any and all claims I may have against WARRIOR CAGE GRAPPLING, the tournament or EVENT VENUE, and any affiliated parties, declared or undeclared including their instructors, officers, agents, servants, contractors, or volunteers, for any injury or damages that may arise during or after my participation in this competition, event, seminar, or class. I also agree to follow all the rules and regulations set forth by the tournament. 

Videography & Photography Release: I hereby consent to the photographing, audio recording, or filming of my participation in WARRIOR CAGE GRAPPLING by WARRIOR CAGE GRAPPLING or third-party media. I understand that these images and audiovisual recordings may be used for promotional, educational, or commercial purposes without any compensation to me. I hereby waive all rights to inspect or approve the finished product and any matter included therein nor solicited. I have carefully read the foregoing release and understand its contents. 

I understand once submitting this RESERVATION I agree to compete if selected by the WARRIOR CAGE GRAPPLING team and that the drop out policy is therefore in effect as well as the late arrival policy. I understand the location of where the event is hosted and have appropriate transportation to attend on time. I understand I may only bring one pre-registered coach and no other unpaid guest of any kind without express permission given.

I agree to this release voluntarily and without exception on the date I submit this RESERVATION.
